Many people confuse acknowledgements with jurats, but they serve completely different purposes. A New York State jurat form (also called a verification upon oath or affirmation) is used when the signer must swear or affirm that the contents of the document are true. This is common for affidavits, applications, and certain government forms.
The key difference: the signer must take an oath or affirmation in front of the notary for a jurat. Simply acknowledging their signature is not enough.
