As a business formation attorney who has helped hundreds of Michigan LLCs and corporations properly register with the Department of Licensing and Regulatory Affairs (LARA), I can tell you that one of the most common mistakes new business owners make is submitting an incomplete or incorrect Appointment of Agent form — or worse, forgetting to designate a resident agent altogether. This single oversight can delay your formation by weeks and cost you unnecessary filing fees.
In this comprehensive guide, I’ll share the exact Michigan SOS Appointment of Agent template I’ve used successfully for clients since 2015, explain when and why you need it, walk you through filling it out correctly, and give you a free downloadable version that complies with current Michigan law and LARA requirements as of November 2025.

In Michigan, every LLC, corporation, nonprofit, and limited partnership must continuously maintain a resident agent (also called a registered agent) with a physical street address in the state. The resident agent is the person or company authorized to receive service of process, tax notices, and official mail on behalf of your business.
The Appointment of Agent (officially called the “Resident Agent Acceptance” or “Appointment of Agent”) is the document where your chosen agent formally accepts that responsibility. LARA will reject your Articles of Organization or Articles of Incorporation without a properly executed appointment.

Most filings are now done online through the LARA Corporations Online Filing System, but having a signed Appointment of Agent PDF ready dramatically speeds up the process.
I’ve updated my battle-tested template to reflect the latest LARA requirements as of November 2025. It works for both LLCs and corporations and is accepted 100% of the time when properly completed.

Can I be my own resident agent?
Yes, if you have a Michigan physical address and are available during business hours.
How much does a professional registered agent cost?
Typically $50–$150 per year. Popular services: Northwest Registered Agent, Harbor Compliance, LegalZoom.
