Since July 2020, you can include the USMCA certification directly on your Canadian commercial invoice template instead of a separate certificate. Include this exact statement (minimum data elements):
“I certify that the goods described in this invoice originate under the rules of origin for CUSMA/USMCA and are eligible for preferential tariff treatment.”
Then list the certifier name, title, company, and date.
