Wisconsin is one of the few states that mandates a specific statutory disclosure form for most residential real estate transactions. Under Wisconsin Statute Chapter 709, sellers of 1–4 family residences must provide buyers with a completed Real Estate Condition Report within 10 days of accepting an offer (unless the buyer waives the right in writing).
The official name of the form is “Real Estate Condition Report” and it is governed by Wis. Stat. § 709.03. The Wisconsin Realtors Association (WRA) also publishes a widely used version that mirrors the statutory requirements but adds helpful instructions.
Key fact: Even if you sell “as-is,” you are still required to complete and deliver the Wisconsin real estate disclosure form. “As-is” does NOT exempt you from statutory disclosures.
