A proof of residency letter (sometimes called an affidavit of residence) is a signed statement from someone who can verify that you actually live at the address you’re claiming — usually a landlord, family member, or close friend who receives mail at that address. The IRS explicitly allows certain residency affidavits for tax purposes (see IRS Publication 519), and many state DMVs have adopted similar formats.

The New Mexico Motor Vehicle Division (MVD) is one of the strictest in the country. According to the official MVD website, you must provide two documents proving NM physical address. If you cannot provide bank statements, pay stubs, or utility bills in your name, New Mexico explicitly accepts a “certified/notarized letter from a person with whom you reside” on their proof of identity/residency checklist.

Louisiana updated its REAL ID requirements in early 2025. The Louisiana Office of Motor Vehicles now accepts an “Affidavit of Residency” executed by the person with whom the applicant resides when traditional documents are unavailable. The affidavit must be signed in the presence of a notary or two witnesses.

West Virginia Division of Motor Vehicles publishes an official WV Affidavit of Residency Form (DMV-3-TR) on their website, but many county clerks and notary offices run out of paper copies. My version mirrors the state form exactly while adding fillable fields so you don’t have to hand-write everything.
The WV affidavit of residency must be notarized and include two proofs of the resident’s own address (utility bill, tax record, etc.). I always recommend attaching those documents as exhibits.

The South Carolina Department of Motor Vehicles (SCDMV) allows a “signed letter from the person with whom you reside” as one of the acceptable documents for proof of SC address. The letter must be dated within the last 90 days, on letterhead if possible, and accompanied by that person’s proof of address.
