Many churches still use physical envelopes or online forms. Here is my simple, compliant donation form template for church use:
For non-cash gifts over $500, the donor must file Form 8283, and gifts over $5,000 require a qualified appraisal. Your church donation receipt letter should only describe the property (“2012 Toyota Camry, VIN #…”) and never state a value.
