The BSA Activity Consent Form, commonly called the “Boy Scout permission slip” by parents and leaders, is the standardized permission and medical authorization document required by the Boy Scouts of America for virtually every troop or pack activity that involves transportation or lasts longer than a few hours.
According to the official BSA National website (scouting.org) and the 2025 Guide to Safe Scouting, the Activity Consent Form combines three critical functions:
This form replaced the older three-part Annual Health and Medical Record (Parts A & B) for most routine activities starting in 2022, which is why you may still see leaders searching for “BSA Activity Consent Form 2022” even three years later.

Clearly state the event name, dates, location, and unit number. Pro tip from experience: Include the departure and return times AND location – this prevents 90% of parent phone calls.
Full legal name, date of birth, and current grade. Many councils now cross-check this against My.Scouting.org records.
The most critical part. Without a physical or qualified electronic signature, the Scout legally cannot participate in most activities (see BSA Barrier to Abuse policies).
Must include policy number and carrier. In emergency rooms, doctors will ask for this exact information. I always recommend parents attach a photocopy of the insurance card to the back.

Yes. The exact same Form 680-701 is required for all BSA programs ages 5–20.
Can I use an electronic signature?
Only if your council has explicitly adopted DocuSign or Adobe Sign through the official BSA portal. Handwritten or typed “/s/” signatures are still rejected by most camps.
Is the Activity Consent Form the same as the Annual Health and Medical Record?
No. The AHMR (Parts A, B, C) is still required for events longer than 72 hours or with higher risk (Philmont, Florida Sea Base, etc.). The consent form is for everything else.
