A credit check authorization form (also called a credit report authorization form or background credit check form) is a standalone, written consent that allows a landlord, property manager, or lender to obtain a consumer credit report, criminal background check, or eviction history on an applicant.
Under the federal Fair Credit Reporting Act (15 U.S.C. § 1681b), you may only pull a consumer report if you have permissible purpose and written authorization. Without a clear, signed tenant credit check form, you risk FCRA violations that can cost $100–$1,000 per violation in statutory damages plus attorney fees (see FTC summary of FCRA).
In my experience representing both landlords and tenants, the single most common FCRA lawsuit trigger is pulling credit without proper authorization language.

A “repull credit authorization” refers to a second (or subsequent) credit inquiry after the initial pull – most commonly requested by SBA lenders during 7(a) or 504 loan processing.
SBA lenders frequently repull credit 30–90 days after the original application to verify no new derogatory items appeared. Many borrowers on r/SBA and Reddit report being asked for a new signed authorization letter titled “Repull Credit Authorization” or “SBA Credit Pull Authorization Form.”
Without this second authorization, the lender cannot legally obtain the updated report.

No. The FTC and multiple court cases require a standalone disclosure.
Do I need a new form for each property?
One form can cover multiple properties if the language says “any rental property owned or managed by Landlord.”
